Instructions
A requirement for all DoD and DVA customers is that each must enroll in this program by selecting the task order agreement which they opt to utilize and complete block 14 (address with DoDaac), (include phone number), block 19 (Explanation of your selection), block 27b (signature), block 27c (date), block 27d (print name).
